There is no form on this page yet, and nothing stands in for one, because a placeholder that looks like a form invites somebody to type an opt-out into a field that goes nowhere. Five things are needed before it can be built: the form provider, the form itself, the list of fields and which of them are required, the copy a visitor sees on submission, and where they land afterwards.

Until then, use the email route above, or the prepared opt-out email, which opens a message already set out with the details Prospero asks for. Every marketing email also carries an unsubscribe link, and requesting erasure is a separate action from unsubscribing: the Privacy Policy sets out both.
